I applied online and heard back from a recruiter the very next day. We had a quick initial phone chat to set up my interview in Cleveland. The recruiter asked me a few easy questions like "what made you choose this position" or "tell me about your internship/classes". They then cover your travel expenses (mileage or flight if you're coming from far away) to get to Cleveland. Once in Cleveland, there's a "Welcome Reception" for all the candidates interviewing that day. You should treat this like an interview – some people even send a follow-up email to the program director. They didn't directly ask about my age or marital status, but they were tricky about it. One interview was pretty bad, with a woman and a man. The woman was rude and condescending from the start. Other candidates had similar experiences with her, so maybe she was testing how we'd handle arrogance.
